To cover administration costs, the Doctors charge £17.50 to be a signatory on a Passport form and £12.50 to be a signatory on a Driving Licence application. They will also charge £12.50 to write a private certificate of state of health (i.e. a ‘sick note’). All patients requiring these services must pay the fee, regardless of financial situation. Payments must be made to the reception in cash, and receipts will be provided.
Private consultations (those that are Non-NHS) will be charged a minimum of £30, to be paid to the reception in cash prior to the consultation.
