| PRIVATE SICKNESS CERTIFICATES: |

| If your employer requires a sick certificate for a period of less than 7 days off work then a private certificate will be issued. You will be asked to pay a set charge for this certificate. |

| DOCUMENTS: |

| To cover administration charges, if you require your GP to complete a form for a passport application or driving licence then you will be charged for this service. |

| PRIVATE MEDICALS: |
| |
| A charge will be applied for private medical examinations e.g. DVLA heavy goods licence medical. The amount will vary depending on the extent of the examination. |

| PRIVATE CONSULTATIONS: |
| |
| These will be charged at a minimum rate of £30. You will be asked to pay the charge at reception prior to your consultation. |

| OCCUPATIONAL HEALTH CHECKS: |
| |
| If you require Blood tests, vaccinations and medical forms to be completed to carry out your work (e.g. Health Care workers) there will be an administration charge for this service. In addition there is the cost of certain vaccinations such as Hepatitis B vaccinations. Please contact reception staff for more information. |

| All payments must be made in cash to reception and written receipts will be provided. |
