| Registration |
|
To access our services you must be registered with the practice. We are currently accepting new patients, provided that you live within our catchment area. Please contact reception staff for details of our catchment area.

| You can pick up new patient registration forms from the reception desk. |

In addition, if you are a UK resident we will need to see proof of address (e.g. a utility bill with your name and address on it) or your NHS Medical Card.
If you are NOT a UK resident, then please inform the reception staff and they will inform you as to the NHS services you are entitled to.
Please note that after completing your forms, it may take a few working days to formally register you with the practice.
Once registered you may be invited to make a 10 minute appointment for a New Patient Check, please bring a urine sample with you to be tested (bottles are available from reception). This is a routine screen and simply checks your general health and past medical history.
REGISTERING YOUR BABY – Once your baby is born then it is important that you register the baby as soon as possible in order for their details to be entered on the system before they have their 6-week baby check and are due to start their immunisations at 8 weeks. Please speak to reception as baby registration forms differ from adult forms. |
